Are license cards required for forklift truck operators?
There is no requirement in the OSHA or ANSI Standards for operators to carry a license card while operating a powered industrial truck.
The documentation requirement as stated in OSHA 1910.178 (l)(6) is, "The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation."
The documentation requirement as stated in ANSI B56.1 is simply, "Appropriate records shall be kept."
Both of these are understood to mean that each employer must maintain a list of their employees and the dates that they were trained and evaluated.
We do supply plastic credit card style license cards as evidence of completion of training. These cards are provided to the employer. It is the employer's responsibility to issue the cards to the employees. In doing so, it is the employer who is certifying, licensing, or authorizing the employee to operate the equipment in the workplace. We, as the trainer, do not "license" anyone. It is the employer who "licenses", "certifes", or authorizes the employee to operate the equipment in the workplace.
For individuals who wish to have operator safety training on any vehicle type, we supply a Record of Training card as shown here.
We do not supply an "Operator Permit Card" or "license card" to individuals whom are not affiliated with an employer.
"Operator Permit" cards or "license" cards are issued by employers to their employees as evidence of the employer's approval, certification, or authorization for the employee to operate the equipment in the workplace.
If an individual, who is not affiliated with an employer, desires operator safety trainng, we will supply an appropriate Record of Training card.
When the individual becomes employed, the employer may contact us for information on the training program that the individual participated in. If the employer desires to have a license card issued for the employee, we will supply the card to the employer for issuance to the employee.
